Since entry permission for " AteloGene® " import is required in many countries, please confirm the attention hereinafter before you place an order.
Atelocollagen, which is the major component of " AteloGene® ", is purified from bovine skin. A lot of countries limit the import of bovine collagen. So we would like you to ascertain whether the permit is required by your government to import the products or not. If you need the permission to import, you need to prepare the documents necessary for the import procedure on your side.
After our confirmation of your entire documents, we will be sending you the product.
When you purchase " AteloGene® ", we need to ask you to submit a written consent (A) and to send the ordering mail (B) to our company.
(A) Agreement for purchasing " AteloGene® "
Please accept all the following items:
| (a). | I will not use " AteloGene® " for testing in human beings. |
| (b). | I will not use " AteloGene® " for applying new patents without Koken's written consent. |
| (c). | I will not use, deliver, sell, assign or let use " AteloGene® " to the person, or the organization which is beyond the scope of my control and responsibility without prior written consent from Koken. |
Please download and print the agreement form (PDF file), after filling in for required matters, and then fax to our company before you place the order.
(B) Placing the Order
After your confirmation of the procedure about import, please give us electronically your ordering information via the “Contact” form which you can reach by clicking the tab on top of any pages.
Name, Association, Position, Address, Phone Number, Fax Number, Destination (in case that the destination is different from your address), products to purchase (#1392 Local Use and/or #1393 Systemic Use) and amounts.
